- About This Program
Dayton Outpatient Center, Inc. (“Dayton Outpatient Center,” “we,” “us,” or “our”) offers a recurring text messaging (SMS) program for patients of our Dayton and Cincinnati locations. These Terms & Conditions govern your enrollment in and use of that program. Please read them carefully. By opting in, you agree to these Terms.
- Messages You Will Receive
Enrolled patients receive non-marketing text messages related to their care relationship with Dayton Outpatient Center, including:
• Appointment reminders and confirmation
• Scheduling and rescheduling messages
• Billing-related updates
We do not send marketing or promotional text messages through this program.
- How You Opt In
You opt in to text messages by providing your mobile number and checking the optional consent box on the Dayton Outpatient Center Text Message (SMS) Consent Form, which we provide when you become a patient. Opting in is optional and is not required to book an appointment or receive care. If you do not opt in, we will reach you by phone. When you opt in, we send one message to confirm your enrollment.
- Message Frequency
Message frequency varies.
- Cost
Message and data rates may apply. Dayton Outpatient Center does not charge for the messages, but your mobile carrier’s standard rates apply. If you have questions about your text or data plan, contact your mobile carrier.
- How to Opt Out
You can cancel at any time by replying STOP to any message. You may also reply CANCEL, END, QUIT, UNSUBSCRIBE, or REVOKE. After you opt out, you will receive one final message confirming that you have been unsubscribed, and you will no longer receive text messages from this program unless you re-enroll.
