This Privacy Policy describes how Dayton Outpatient Center, Inc., and its subsidiaries and affiliates (collectively, “Company”, “we”, “us” or “our”) will collect and use information that you provide, and information collected from our website, mobile applications and data transmitted to Company (the “Services”). What is Covered by the Privacy Policy?

We may also use certain third-party products or services including RingCentral, Facebook (Facebook Pixel), Instagram, and Google (Google Analytics), in order to better understand the use of our Services. The Third-Party Tools may use the data collected to track and examine the use of the Services, to prepare reports on its activities, and to share them with other related services. Third-Party Tools may also use the data collected on the Services to personalize ads on their own advertising networks. The ability to use and share information it collects about your visits to the Services is restricted by the third-party privacy policies and terms of use.

Your Control Over Information

Information We Share

We may share information with a subsidiary, or affiliate entity within Company as permissible under HIPAA. Any organization to which we provide this information is required to keep it confidential and comply with this Privacy Policy.

We may share information with service providers and business partners who perform a service for us. For example, we may provide information as necessary to companies that host our Services or send communications on our behalf. These service providers may have access to information about you if it is needed to perform the services, but they are not authorized by us to use or disclose the information except to perform services or as required by law and they are required to maintain the confidentiality of the information.

Business Partners

Company may share your information with our business partners, such as local advertisers who pay for banner ads on our Services. Short Message Service (SMS) Terms and Conditions

When you sign up for text messages from the Company you are signing up to receive text messages related to your relationship with Company and its subsidiaries and affiliates. This may include messages related to: scheduling your appointments or billing questions. You can opt-out of SMS messages by texting STOP to respective short code. Your opt-out request will generate one final message confirming that you have been unsubscribed. You will no longer receive SMS messages from the short code you opted out from.

California Privacy Rights

California residents have the legal right to request in writing from businesses with whom they have an established business relationship: (a) a list of the categories of personally identifiable information such as name, email, and mailing address, and the type of services provided to the customer that a business has disclosed to third parties (including affiliates that are separate legal entities) during the immediately preceding calendar year for the third parties’ direct marketing purposes; and (b) the name and address of all such third parties.
