Referring Your Patients to DOC
When you send your patients to DOC Pain Management, they are connected with one of our friendly patient coordinators, who work to obtain all the necessary documentation for our physicians to provide you and your patients with a comprehensive diagnosis and treatment plan. We strive to make this process as easy as possible for all parties.
Referrals can be made through the electronic medical record, by referral form, or by contacting Jessie or Sarah (New Patient Coordinators) at 937-252-2000 Ext 358.
